Care
The Interim HealthCare staff assure complete
continuity of care for your patients. Before discharge, the
Director of HealthCare Services or Home Care Supervisor will
confer with the discharge planner and the child's physician,
as well as the entire family in the home environment.
Based on professional instructions, individual
abilities and needs, the Interim HealthCare team will develop
a comprehensive care plan. Using our exclusive Skillmatching
system, we select personnel whose skills uniquely suit the
specific home care requirements. We can coordinate all the
details, including necessary equipment and supplies.
At discharge, we will be present to assist and to accompany
the patient home if appropriate.
Once in the home, you can be assured a smooth transition
as the home care plan is implemented. The assigned Interim
HealthCare team will provide specialized treatment with professionalism
while maintaining clinical records.
